01/05 - Present - Executive Director of the Central Valley Health Policy Institute and Faculty, Department of Public Health, California State University, Fresno
As Executive Director I am responsible for the management of the Institute, in coordination with the Dean of the College of Health and Human Services and the Director of the Central California Center for Health and Human Services. I initiate, cultivate and facilitate relationships between the Institute and University faculty, other academic and non-academic health researchers, state and county health departments, and public and private research institutes. I coordinate the activities of collaborating units and faculty with the university, provide methodological data management, proposal development input and support to collaborating faculty, and act as spokesperson for the Institute regarding health policy research and activities. I am also responsible for identifying opportunities and collaborative ventures that will facilitate securing of external funding for Institute sustainability, coordinating with regional partnerships and institutions in the development of information databases, facilitating coalitions among health and community organizations, and for ensuring program sustainability and recognition.
As a Professor in the Department of Public Health I have been responsible for curriculum development for the new Health Policy and Administration option in the Master's in Public Health program. I have also been instrumental in coordinating with department faculty for the implementation of the new option.

As Director of Long Term Care Studies, I have been responsible for development of sponsored research, policy analysis, and technical assistance projects. I have been Principal Investigator or other lead roles on more than 30 projects with combined total budgets over $10 million. These projects have often required managing large budgets and complex staffing or project timing, building and maintaining relationships with diverse health sector relationships, and design and interpretation of demonstration and evaluation projects. As a practitioner and teacher, I encourage people to integrate multiple ways of knowing and thus attempt to blend quantitative, qualitative, and archival data in evaluations and policy-relevant, theory-based social science projects addressing health and health care broadly defined. I have been formal or informal mentor to many younger scholars and continue to be involved in my school and nationally in supporting the emergence of a new generation of health researchers. As Professor at the Heller School and Co-Chair of the Social and Economic Inequalities (SEI) Concentration, I have lead or participated in committees on diversity, institute management, faculty and staff renewal, student admissions and planning community outreach. I have played a lead role in developing the concentration, shaping its course selection, and teaching its first-level course, one of the required courses for doctoral students.

As a Health Policy Analyst for the Virginia Center on Aging, I facilitated and provided technical support to the Virginia Governor's Council on Aging and worked closely with other aging services organizations. I also coordinated studies evaluating health care reforms within the state in close collaboration with state government, participated in national level research on health services for older people and adolescents, and taught classes on health services research methods.

At Berkeley Planning Associates I was hired initially as an analyst on a large-scale Federal evaluation of 16 Medicaid or Medicare demonstrations (including some California programs such On Lok in San Francisco and the statewide MSSP) of coordinated community long-term care. Because of several retirements, at 27, I found myself responsible for a multimillion-dollar evaluation project, a staff of 20 people, and a range of subcontracts and data agreements. The findings of this project continue to be well-known to long-term care researchers and many of the issues we addressed continue to be debated.

- Courses Taught
- As a senior faculty member at the Heller School, I was responsible for the development of our concentration in social and economic inequalities. I facilitated the ongoing Seminar of Social and Economic Inequalities that offered weekly opportunities for students and faculty to explore shared interests with emerging challenges in inequality and their implications for social policy.Since, 1997, I taught Population Groups and Social Policy a required course for PhD students on how race/ethnicity, gender, social class, age, and health status are conceptualized and studied by human service policy researchers and the policy makers. Since 1998, I taught Race/ethnicity and Gender in Applied Research every other year. This course provides PhD and MBA students with a practical perspective on conducting, evaluating and applying the findings of quantitative and qualitative research on disparities in health and other outcomes. Since 1995, I taught, on an occasional basis, Meta-Analysis in Health Services Research , a course that introduces students to advanced statistical methods for summarizing research and exploring the boundary conditions on observations, Long-term Care Policies and Programs , which provides a thorough introduction to eldercare research and programming, and Diversity Management in Human Service Organizations a class that introduces management students to multicultural theories and their application to management.
Dr. Capitman is an accomplished speaker and he has conducted numerous presentations to diverse groups during 2005. The following are highlights of his accomplishments:
- Two presentations on the role of the Central Valley Health Policy Institute to staff of the VAPHER Institute at University Medical Center and the Fresno Business Council Air Quality Forum.
- Presented a paper, New Research in Asthma Epidemiology: Implications for Health Policy, at the VAPHER Institute Conference.
- Three presentations on the Health Policy Leadership Program and the Interdisciplinary Health Policy Fellowship program for faculty at all three state universities in the Valley - Fresno Stanislaus, and Bakersfield.
- Presented a paper that the 26th Annual Central California Research Symposium on April 21, 2005. His paper was entitled Birth Outcomes in the San Joaquin Valley
- Presented, in conjunction with CVHPI staff, a policy briefing to the California Legislative Rural Caucus in Sacramento on the impacts of the proposed Medi-Cal Redesign.
- Presentation at The California Endowment's Agricultural Workers Conference in Sacramento on Policy and System Analysis for Change.
- Spoke on the Insight radio program on the topic of Quality of Life in the San Joaquin Valley. He specifically spoke on the impacts of healthcare financing and delivery systems in the region and healthcare outcomes, as well as the role of racial/ethnic and social class disparities in shaping overall the well being of the region.

Grants and Proposals
Principal Investigator: Home Garden Needs Assessment Project funded by The California Endowment (2006). This project was designed to provide the community of Home Garden, near Hanford, with a community needs assessment relating to health care and access needs.
- Principal Investigator: Elders of Color with Dementia in Senior Housing, funded by the Alzheimer's Association (2005). This project includes a longitudinal nationwide survey of African American, Latino/Hispanic, and Asian/Pacific Islander elders diagnosed with dementia. The project examines the effects of community supportive services and cultural competence initiatives on maintenance of community tenure.
- Principal Investigator: Adoption of Substance Abuse Screening and Early Intervention for Adolescents Served in Federally Qualified Community Health Centers, funded by National Institute for Drug Abuse (2005). This study uses a national survey of medical directors of Community Health Centers to understand the factors that lead to adoption of screening and early intervention protocols around tobacco, alcohol, marijuana and other drugs.
- Principal Investigator: Cultural Explanatory Models of Mammography Use, funded by the Susan B. Komen Foundation (2004), this study combines qualitative and quantitative data on a multi-ethnic sample of low-income women in Massachusetts to examine the determinants of mammography use. A number of alternative measures of use and alternative approaches to including the cultural explanations of cancer are being examined.
- Co-Principal Investigator: Center for Medicaid and Medicare Services Project to Eliminate Racial Disparities in Cancer (2000-2003). Sarita Bhalotra MD PhD Co-PI. With 23 staff and multiple sub-contracts, this short-turn around project included a comprehensive review of the published literature on racial/ethnic disparities in cancer risks, primary prevention, screening, treatment and outcomes; telephone interviews with 117 emerging programs addressing same issues but not reflected and in-depth case studies of 28 programs nationwide. We developed and worked with the government to refine place for design and evaluation of demonstration projects. In several current projects funded by CMS and performed with Tennessee Quality Improvement Organization, I am leading projects to identify additional clinical areas in which disparities arise and additional population segments that should be targeted in disparity reduction initiatives. In a related project, I am Co-Investigator for a multi-year Evaluation of Lifestyle Modification Programs (2000-2005).
- Co-Principal Investigator: Robert Wood Johnson: Substance Abuse Policy Research Program Evaluation of Exxon-Mobil Agreement for Voluntary Compliance. (2004 - 2005). With my colleagues Brad Krevor, PhD and Mathilda Ruwe, MD, I have been studying how to control youth access to tobacco products and have completed several projects in this area, such as a field evaluation of electronic age verification devices. In this study, we examine the experiences of 900 Exxon Mobil stores as they seek to implement policies and procedures to ensure that their personnel don't sell tobacco products to kids. Other projects in this areas have included: Co-Principal Investigator: Evaluation of the Robert Wood Johnson Foundation's Addressing Tobacco in Managed Care. This (1997 - 1999) project examined the efforts of managed care organizations to adopt smoking cessation protocols. Co- Principal Investigator: Application of the Chronic Care Model to Smoking Cessation in Primary Care. This project developed a self-assessment tool for primary care settings to test the adequacy of their office systems of resources and mechanisms to support interventions with smokers.
- Co-Principal Investigator: Evaluation of the Robert Wood Johnson Foundation's The Access Project. In this project, I worked with Jeff Prottas, PhD (1998-2001) and others to provide ongoing internal evaluation support to the Access Project. We tracked participants from community-based health care advocacy organizations that used technical assistance materials or attended workshops.
- Director: National Resource Center Diversity and Long-Term Care (1989-1996). This project provided technical assistance to state and local agencies on aging and other provider organizations in developing systems of care and treatment approaches that were responsive to racial/ethnic and gender differences in needs and preferences for long-term care. With a large staff, numerous subcontractors, and a broad range of workshops, research projects, and communication platforms, this program built on emerging knowledge in eldercare to support local communities and national initiatives in serving elders of color. The National Resource Center was inspired by and provided outlets for information gleaned from a range of community long-term care projects. Among these were: Principal Investigator: Office of Technology Assessment Study on Autonomy and Long-Term Care (1990-1992) explored the ethical issues in community care and the opportunities for expanding the power of elders themselves in shaping the long-term care system; Co-Principal Investigator: Hartford Foundation Future of Long-term Care Study (1991-1993) explored the potential for national policy and programs based on the experiences in community care demonstrations; Co-Principal Investigator: Health Care Financing Administration Initiative to Plan Phase II of the Social Health Maintenance Organization, was one of a number of studies (including ongoing work) to describe experiences of participants in these capitated and integrated acute and long-term care programs (including a site in Orange County, CA) for national policy; Principal Investigator : Health Care Financing Administration Study of Quality Measures for Home Care (1993 - 1995) developed performance measures for community based eldercare based on the concept of unmet needs for services; Principal Investigator: Evaluation of the Robert Wood Johnson Foundation's Hospital Initiatives in Long Term Care (1989-1991) examined the potential roles of acute care hospitals in meeting the chronic care needs of older people; Principal Investigator: Evaluation of the Robert Wood Johnson Foundation's Dementia Care and Respite Services Program (1992-1995) assessed the cost-effectiveness and organizational impacts of new services based in adult day centers targeted to persons with dementia; Co-Principal Investigator: Commonwealth Foundation Study of Assisted Living (1995 - 1998) conducted the first national survey of these housing +services programs to consider both consumer and provider perspectives on quality of care.
- In December 2005 CVHPI staff has completed a subcontract with California State University, Fullerton to provide them with a comprehensive list of all schools, both public and private, with enrollment information by grade, addresses, and school calendar. As part of the sub-contract CVHPI is also facilitating community forums throughout the region to discuss the findings from this large environmental impact study. The forums are scheduled for spring 2006.
- Pending Proposals
Principal Investigator: PINK-FAST Patient Inclusion, Navigator Knowledge - From Abnormal Screening to Treatment In collaboration with The San Joaquin Valley Health Consortium (SJVHC), Central Valley Health Policy Institute (CVHPI), California State University, Fresno (CSU, Fresno) and University Medical Center (UMC) are proposing PINK-FAST as a California Breast Cancer Community Research Collaboration pilot project to explore the feasibility of a breast cancer navigator (BCN) program at the safety-net cancer care facility in Fresno County, to address racial/ethnic and insurance status disparities in breast cancer (care, morbidity and mortality. Hispanic, Asian/Pacific Islander, Hispanic and African-American women generally have more advanced stages and poorer stage-specific survival than their non-Hispanic, White counterparts. These outcome disparities reflect racial/ethnic and insurance status-related differences in completion of the cancer care sequence from "suspicious" mammogram results through definitive diagnosis, primary and secondary treatment, aftercare and monitoring. While a number of published studies indicate the potential effectiveness of using breast cancer navigators to assist women in completing the cancer care process sequence, none have used a controlled experimental approach to assessing the clinical impacts and cost-effectiveness of such interventions. PINK-FAST, therefore, features a formative evaluation. The central question to be addressed in PINK-FAST, is how best to design and implement an effective BCN program for traditionally underserved women in Central California. The specific aims are:
- Identify the points in the cancer care process, at the safety net cancer care provider in Fresno County, where R/E and health insurance coverage disparities occur.
- Assess patient, practitioner, facility and community determinants of disparities in BC care in order to identify clinical process changes and new resources necessary to reduce these disparities.
Co-Principal Investigator : The goal of The Children's Health Initiative for Fresno County Analytic Grant Proposal is to develop the infrastructure and capacity to conduct a formative evaluation of the Children's Health Initiative for Fresno County (CHI). This project will direct program activities and resources towards meeting the program's objectives to 1) maximize families' participation in available public programs in Fresno County; 2) increase the number of insured children in Fresno County; and 3) optimize revenues to ensure children's access to health care services.
Principal Investigator: The proposed project will demonstrate and evaluate effects on asthma knowledge, quality of life, and healthcare outcomes of a Visually Enhanced Interactive Learning (VEIL) approach to asthma patient education at two urban safety-next primary care sites that serve diverse Latino, Haitian, and Southeast Asian populations. Both standard care and VEIL asthma education programs in the proposed study will be based on NAEPP guidelines and be delivered by ethnically-matched health educators. VEIL health educators will also provide structured feedback to primary care clinicians. Specifically, the proposed controlled clinical trial will use a multi-ethnic population of children and adolescents (ages 12-17) with mild persistent or more severe asthma to:
1) Test the impact of VEIL asthma education in comparison to standard care asthma education on patient asthma knowledge;
2) Test the impact of VEIL asthma education in comparison to standard care asthma education on patient quality of life, lung function, and healthcare utilization;
3) Test the impact of VEIL asthma education on primary care practitioner a) accuracy in assessment of patient knowledge, and b) communication effectiveness;
4) Describe the asthma and its management concepts and barriers to asthma management for low-income children and adolescents in immigrant families.
Co-Principal Investigator with Dr. Derek Shendell: Submitted a grant proposal to the National Institutes of Health entitled, Assess Adult Environmental Health Literacy at Schools: A Community-Based Project. This community-based, participatory environmental health literacy pilot study is comprised of two specific aims.
- Examine the feasibility of planning, recruiting targeted participants--school nurses, and teachers of 4th, 5th and/or 6th grade students in targeted schools in participating communities in SJV counties-- through local asthma coalitions and schools, and conducting two-part EHS literacy workshops.
- With a one-page, two-sided, self-administered survey instrument based on previously validated questions from various sources that would take most people 5-10 minutes to complete (up to five minutes per side), in English and in Spanish, assess the basic knowledge/awareness of EHS (baseline versus after workshop) of school nurses and teachers, and evaluate this pilot study's workshops.
Co-Principal Investigator with Dr. Derek Shendell: Submitted a grant proposal to the NIH. The proposal, Community-Based Pilot Study: Attendance and Reason for Absence, Tardy and Leave Early, has two specific aims:
1. To further develop, and test for one school year, a simple spreadsheet-based methodology, previously proven in a school-based study in the Pacific Northwest), for collecting information on school attendance that would be enhanced to distinguish between all students and diagnosed asthmatics, and possibly record reasons for absence like manageable illnesses like asthma.
2. To develop, and test for one school year, a new, simple checklist (spreadsheet)-based methodology for collecting information on student tardiness and leaving early from school, data not captured by enrollment and daily attendance metrics, and reasons among those identified in collaboration with members of multiple local asthma coalitions in Central California's San Joaquin Valley.
Co- Principal Investigator with Dr. Van Nee VanVleck: The proposal, Hospital Finance in California's San Joaquin Valley: Developing a Simulation Model to Assess Medi-Cal Redesign, was submitted to the California Program on Access to Care, California Policy Research Center, University of California, Office of the President. This study will examine the relationships between revenue, revenue mix and utilization patterns for San Joaquin Valley (SJV) hospitals and if these patterns differ from California as a whole or from "peer" regions within California. It will also explore how alternative policy initiatives such as Medi-Cal cost-containment, hospital finance restructuring and expansion of insurance access influence the financial performance of SJV hospitals.
